Glasses Habits Survey 2026 — $50 Frame Credit Draw
The official rules for the prize draw attached to the 4 Eyes Club Glasses Habits Survey 2026. No purchase is necessary to enter or win.
Sponsor. Go Optical LLC, 75 Summit St, Manchester, CT 06040 — publisher of 4 Eyes Club (4eyesclub.com) — provides the prize.
The prize. One (1) frame credit of $50, redeemable at Go Optical LLC, 75 Summit St, Manchester, CT 06040, toward a frame purchase. The credit has no cash value, is not transferable, and must be redeemed by June 30, 2027.
Eligibility. Legal residents of the United States who are 18 or older at the time of entry. Void where prohibited.
Entry period. October 20, 2026 through November 3, 2026 at 11:59 PM Eastern Time.
How to enter. Either (a) complete the Glasses Habits Survey 2026 and leave an email address on the final question, or (b) email hello@4eyesclub.com with the subject line "Survey draw entry" during the entry period. One entry per person, whichever route you use. Completing the survey is not required to enter.
The drawing. One winner will be drawn at random from all eligible entries on November 30, 2026. The odds of winning depend on the number of eligible entries received.
Winner notification. The winner will be notified by email within seven days of the drawing and must reply within 14 days to claim the prize. If the winner does not reply in time, an alternate winner may be drawn.
The 10% code is separate. Every survey respondent who finishes sees a 10% discount code for GoOptical.online on the completion screen. That's a participation thank-you, not a prize — it isn't part of this draw and doesn't depend on entering it.
